Appointments
Please call 352.345.4876 to schedule an appointment with our office. Our staff will need your current address, phone number, insurance information and nature of the visit each time you call. You may also download and fill out the patient forms prior to the initial office visit to expedite the appointment time.
Appointment Cancellation Policy
If you are unable to make your appointment, please call our office at least 48 hours prior to the appointment time to allow us to utilize this time for another patient.
Insurance and Payment
We participate in most major insurance plans*. As a courtesy to our patients, we file with the insurance companies with which we participate. Your insurance requires that we collect your copayment and any deductible at the time service is provided.
If you participate in an HAS, HRA, or FSA, we will verify with your carrier your responsibility prior to your visit. This amount will be due at the time service is provided.
We accept cash, MasterCard, Visa, Discover, debit cards and personal in-state checks.
